A manufacturing plant in Oldcastle has been penalized $185,000 due to a workplace fatality that occurred nearly two years ago.

As reported by the Ontario Ministry of Labour, Immigration, Training and Skills Development, a worker lost their life on Aug. 12, 2024, at Integrity Tool and Mold Inc., a company that supplies moulds for the automotive sector, located on Devon Drive.

CBC Windsor attempted to contact Integrity Tool and Mold Inc. for their input but did not receive a response before the publication deadline.

The ministry states that the worker was fixing a CNC machine – an automated tool guided by pre-programmed software designed to accurately cut, shape, and drill materials like metal and plastic.

At that moment, the machine was functioning in manual mode when the worker initiated a program that switched it to automatic mode.

This is when the worker entered the machine enclosure for repairs and unfortunately suffered fatal injuries.

A subsequent investigation by the ministry revealed that the CNC machine had not been powered off or locked out while the worker was inside it. Additionally, they lacked adequate lockout training and were not given locks for the machine.

The inquiry also uncovered that electrical lockout points on the machine were incorrectly labeled and no lockout placard was available.

<p. After pleading guilty in provincial court in Windsor, the ministry announced that the company was fined $185,000. The court also imposed a mandatory 25 percent victim fine surcharge intended for a provincial government fund that supports crime victims.
