The City of Kingston has announced that 33 new transitional housing units at 315 Queen Mary Rd. are set to open in August as the shared care model wraps up construction.

The city acquired the old Extendicare building in 2024 to establish a facility that combines primary health care, transitional housing, and community programs designed to tackle social determinants of health.

The shared care model consists of three parts: transitional supportive housing, a primary health-care clinic, and space for community programming.

The primary health-care clinic, called the Midtown Kingston Health Home and managed by Kingston Community Health Centres, has moved to 315 Queen Mary Rd.

Home Base Housing will run the transitional housing program aimed at adults aged 55 and older with different health-care needs. The program will offer round-the-clock staffing and case management focused on helping residents find more stable housing.

Community programming is expected to kick off later this year with a vocational laundry initiative led by Lionhearts. Set to launch in October, this program will provide hands-on job training and essential employment skills. More community programs should start in early 2027.

On July 7, Kingston city council approved plans for a wellness and health center within the community space of the facility. This center will enhance existing health services by providing respiratory rehabilitation, occupational therapy, physiotherapy, hydrotherapy, and other wellness support options.

The city is also inviting applications for a Community Liaison Committee to facilitate communication between partner agencies and local residents. Applications will be accepted until Aug. 9 on the city’s Get Involved Kingston website.
