Mesa represents a collaborative and caring approach to health – a tailored response from Renfrew County to tackle complex issues like affordable housing, homelessness, mental health, substance use, and addiction. Guided by community needs and real-life experiences and grounded in Mesa principles, local partners successfully applied for a Homelessness and Addiction Recovery Treatment (HART) Hub, which is a regional initiative backed by funding from the Province of Ontario.
In January 2025, the Renfrew County Mesa HART Hub received approval for a three-year investment of $6.3 million from the Government of Ontario to enhance coordinated services for individuals facing homelessness, mental health issues, and substance use challenges. With initial development underway, the HART Hub is excited to announce that two key leadership roles have been filled.
To support this ambitious and collaborative effort, Mesa partners are focusing on investing in local talent with proven experience.
Molly Fulton will step into her new role as HART Hub Lead on a three-year secondment from her position as Manager of Community Mental Health at Pembroke Regional Hospital. She brings extensive knowledge along with a strong dedication to enhancing local mental health systems.
The Renfrew County Mesa HART hub Lead will oversee all operations, strategic direction, and management within the Renfrew County Mesa HART Hub. This role acts as a central point for aligning health care, addiction services, and mental health resources alongside local stakeholders and community partners. The Lead will guide the hub’s multidisciplinary team to ensure effective service delivery while promoting collaboration and innovative solutions that address community needs.
“We are strengthening our collaborative approach to compassionate care by building on what we know, learning from our work together, and investing in local expertise,” said Sabine Mersmann, President & CEO of Pembroke Regional Hospital-a key partner in the HART Hub. “Establishing the HART Hub Lead position demonstrates our commitment to ensuring that those in need receive timely coordinated care with the best opportunity for success.”
We’re also thrilled to welcome Tara Edeh from the Ontario Addiction Treatment Centre in Pembroke as our new HART Hub Site Manager. This important frontline leadership position will aid in coordinating accessible client-centered paths to care so individuals can get help at just the right time-and find a safe place they can call home.
The HART Hub Site Manager is responsible for managing daily operations at various HART Hub locations across Renfrew County while playing an essential role in executing plans effectively along with performance monitoring-creating a safe environment aligned with what we aim to achieve through our mission.
“Our teams are on the front lines of a new compassionate model of care. The Mesa HART Hub brings us together to deliver more coordinated wraparound supports that lead to safer healthier communities for all,” said Renfrew County Warden Peter Emon.
These two appointments represent significant progress toward fulfilling our vision for the Mesa HART Hub: creating a compassionate local system that meets people where they are-24/7-and supports them on their journey towards recovery stability and overall wellbeing.
Services available through the Mesa HART Hub will include:
24/7 access to immediate supports Primary care and health outreach Mental health and addiction services Peer support and recovery navigation Supportive housing Indigenous client care coordination Life skills and employment counselling
